A woman in her mid-20s was evaluated for a history of dyspnea on exertion for 2 years. A 6F pigtail catheter was placed in the left ventricle via the right femoral artery, but the patient developed acute onset of dyspnea along with retrosternal chest pain; the catheter was pulled back into the right atrium, and chest fluoroscopy was performed.
