One advantage of microneedle fractional RF is that it does not depend on specific chromophore absorption and is not affected by tissue diffraction, unlike laser or light-based devices, and is theoretically safe for patients of all skin types. Compared with fractionated CO2 lasers, microneedle fractional RF may achieve faster healing time, minimal patient downtime, and lower risk of scarring, showing improvement equivalent to or better than that of fractionated CO2 laser. A limitation with this treatment modality is that physicians are blinded to the depth of penetration of RF energy. In addition, skin impedance strongly affects RF current flow, which may vary with patient age, skin color, and treatment location. In this case, microneedle fractional RF improved the aesthetic and functional aspects associated with chronic radiation dermatitis. We hypothesize that microneedle fractional RF achieved these results through collagen and extracellular matrix remodeling, and we propose that this technique may have a role in treating patients with chronic radiation dermatitis. Microneedle fractional RF therapy represents an advance worthy of investigation by clinical researchers to determine whether the benefits presented herein can be reproduced and benefit additional patients with chronic radiation dermatitis.
