Key Points Question Can effective comparison groups be created to assess county-level public health performance? Findings In this cross-sectional study including more than 300 million individuals in the United States, 8 groups of counties with similar sociodemographic characteristics were identified. Percentile ranks for outcome prevalence often differed when counties were considered within their cluster in comparison with a nationwide scale. Meaning Supplying counties with peer groups created using sociodemographic similarities may help local health departments to compare progress and share techniques in efforts to address leading public health challenges.
