Key Points Question Is myelin damage in normal-appearing white matter associated with cognitive impairment in participants with multiple sclerosis (MS)? Findings In this cross-sectional study of 73 participants with MS and 22 age-, sex-, and education-matched healthy controls, significant associations were observed in participants with MS between a quantitative neuroimaging measure of myelin and performance on cognitive tests validated for MS. No significant associations were found between myelin measures and cognitive performance in controls. Meaning These findings suggest that myelin damage that is completely invisible on standard clinical images but can be measured using myelin water imaging is involved in MS-related cognitive impairment.
