Key Points Question Is monthly variation in gastric cancer screening rates associated with changes in rates of cancer detection? Findings This cohort study of 21 535 222 participants from the Korean National Cancer Screening Program database found that the monthly number of endoscopic examinations increased toward the end of the year. However, the gastric cancer detection rates were negatively and significantly associated with calendar month. Meaning The findings of this cohort study suggest the importance of the implementation of policy guidelines to strengthen the merits of the screening program, including a reduction in gastric cancer-related mortality.
