Key Points Question What are the associations of cancer stage at time of diagnosis, treatment, and marital status with cancer-specific survival? Findings This cohort study of 1.7 million adults with cancer found that stage at diagnosis mediated the association of marriage with cancer-specific survival in breast, colorectal, and endometrial cancers as well as melanoma; receiving surgery mediated the association in lung, pancreatic, and prostate cancers; and receiving chemotherapy mediated the association in lung and pancreatic cancers. Meaning These findings suggest that promoting the early diagnosis of breast, colorectal, and endometrial cancers and melanoma and providing definitive treatment for lung, pancreatic, and prostate cancers could improve cancer-specific survival among unmarried patients with cancer.
