Key Points Question What is the association of albuminuria with chronic kidney disease (CKD) regression? Findings In this cohort study of 58 004 adults with newly diagnosed moderate to severe CKD (eGFR, 15-44 mL/min/1.73 m2), albuminuria level was directly associated with CKD progression and death, and inversely associated with sustained improvement of eGFR for 90 days or longer. Five-year probabilities of CKD regression were higher in people with lower urine albumin-creatinine ratios in a stepwise fashion. Meaning These findings suggest that there may be novel opportunities to discuss both favorable and adverse outcomes with adults with CKD.
