Key Points Question Is endovascular recanalization associated with better functional outcome for patients with ischemic stroke who present in the extended time window with extensive signs of infarction? Findings In this cohort study of 5853 patients, the rate of successful vessel recanalization was 75% and was independently associated with a higher rate of favorable outcomes, defined as a modified Rankin Scale score of 3 or less. Meaning The findings of this study suggest that endovascular treatment for patients with ischemic stroke and extensive signs of infarction who present in the extended time window was associated with better functional outcomes, encouraging randomized clinical trials to investigate the treatment of patients with ischemic stroke presenting in the extended time window who also show signs of extensive infarction.
