Key Points Question Is it feasible to assess solid tumor response to treatment from electronic health record (EHR) documentation and how does it correlate with clinical trial standards? Findings This cohort study found that a modified version of RECIST 1.1 with centralized independent readouts (imaging response based on RECIST) enabled patient-level benchmarking of an EHR-based response variable. In a concordance analysis (N = 100), their agreement was 71% and 74% for confirmed and unconfirmed response, respectively. Meaning The use of a modified version of RECIST with independent centralized review was feasible, the concordance between clinician-assessed response and centralized imaging response based on RECIST was moderate.
