Key Points Question Does intravenous thrombolysis (IVT) prior to unsuccessful thrombectomy improve functional outcomes in patients with acute ischemic stroke compared with thrombectomy alone? Findings In this multicenter cohort study of 746 patients with acute ischemic stroke, prior treatment with IVT was associated with improved functional outcomes on the modified Rankin Scale at 90 days compared with thrombectomy alone. Meaning These findings suggest a new perspective to the debate on noninferiority of combined stroke treatment; the subgroup of patients with unsuccessful thrombectomy might particularly benefit from prior IVT, presumably driven by improvements of cerebral macrocirculation and microcirculation.
