Key Points Question Is antibiotic therapy associated with response to chemotherapy in patients with metastatic pancreatic ductal adenocarcinoma (PDAC)? Findings In this cohort study of 3850 older adults with metastatic PDAC, receipt of perichemotherapy antibiotics was associated with an 11% improvement in survival for patients who received gemcitabine. In contrast, there was no association between antibiotics and survival among patients who received fluorouracil. Meaning The findings of this study suggest that the addition of antibiotics to first-line gemcitabine chemotherapy regimens may improve outcomes for patients with metastatic PDAC.
