Key Points Question What is the association of stroke with a new cancer diagnosis after a first stroke for younger and older patients compared with peers from the general population? Findings In this cohort study of 27 616 patients aged 15 to 49 years and 362 782 patients aged 50 years or older, incidence of cancer among the younger group was more than 2 times greater in the first year after ischemic stroke and more than 5 times greater in the first year after intracerebral hemorrhage when compared with the general population. The older group experienced a smaller, but still increased, incidence of cancer. Meaning This study suggests that patients aged 15 to 49 years have a 3- to 5-fold increased risk of cancer in the first year after stroke compared with individuals who have not had a stroke.
