Key Points Question What is the effect of a clinician vaccine communication strategy designed to promote vaccine uptake on the vaccination status of children of parents with negative vaccine attitudes? Findings In this cluster randomized clinical trial involving 937 parent-child dyads at 24 clinics in Washington and Colorado, no significant overall difference was found in percentage of days underimmunized among children at intervention and control clinics, although intervention (vs control) children in Washington had significantly increased odds of receiving all vaccinations on time through 19 months of age. Meaning The communication strategy did not improve vaccine uptake among children of parents with negative vaccine attitudes, although state-specific effects were observed.
