M anticoagulation therapy for patients undergoing surgery can present many challenges for the perioperative team. Each patient has a unique set of comorbidities that affects their risks for bleeding and clotting and must be assessed individually. Perioperative nurses are in a unique position to assist both providers and patients to help ensure the safe and effective management of anticoagulation therapy. Following evidencebased guidelines and providing patients with clear instructions regarding anticoagulation will help ensure the best outcomes for patients.
