W we step into the OR, we know that we are expected to “get it right” for our patients and surgeons every time. We strive to work in a zerodefect environment because we know the outcome of our work can change lives. To perform at the highest level, we need communication to be as flawless as possible. Ineffective communication is one of the most frequently identified root causes of sentinel events that contribute to the 180,000 to 400,000 deaths that occur each year from preventable medical errors.1
