Dual catheters are mandatory in almost all CTO PCI. Since TRA PCI has been shown to be safer than transfemoral PCI because of reduced bleeding complications, using at least one TRA logically halves the risk of access site complication. Dual TRA permits early sitting and ambulation, which can be a huge advantage in some patients. However, the small size of the radial artery remains an important limitation for the use of large-bore catheters (>6 Fr), restricting thereby the treatment of highly complex lesions through TRA.
