• In a retrospective propensity matched cohort, both transapical and transfemoral transcatheter aortic valves demonstrated favorable left ventricular stroke volume index and aortic valve hemodynamics at 1 month. • Localized apical wall motion and strain abnormalities from transapical approach did not affect overall myocardial performance. • Transapical approach may be acceptable in cases where transfemoral access is limited with similar four-year survival for both cohorts.
