• Preprocedure TIMI flow grade has been an important predictor of clinical outcomes including mortality in patients with STEMI. • Results from a substudy of the ATLANTIC trial indicate that while the unadjusted preprocedure TIMI flow grade was related to composite ischemic events at 30 days, it was an independent predictor of only stent thrombosis. • TIMI flow grade is a simple but incomplete measure of coronary perfusion in STEMI patients. TIMI frame count, TIMI myocardial perfusion grade, and the index of microvascular resistance provide more complete insight into myocardial reperfusion in STEMI patients.
