• Valve thrombosis is a rare but relevant and potentially fatal complication of transcatheter interventions (TAVI) that is usually resolved by oral anticoagulant therapy. • Advanced imaging including 4D-CT is pivotal for the diagnosis. • Patientand procedure-related factors that increase the risk of valve thrombosis include obesity, renal failure, thrombocytopenia, valve-in-valve implantation, lack of predilation, and use of balloon-expandable prosthesis.
