• Alcohol septal ablation (ASA) carries a greater risk of complete heart block (CHB) than surgical myectomy. • The authors propose a risk score to help determine patients' individual risk of CHB, using preprocedural, procedural, and postprocedural factors. • These data may help guide temporary pacing time and postdischarge rhythm monitoring of patients after ASA. • Further data are still needed to better individualize CHB risk and perhaps to inform the choice between ASA and surgical myectomy if the risk is low.
