A 41-year-old man with a history of adult T-cell leukemia presented to the hospital with hilar lymphadenopathy and a right upper lung mass. Endobronchial ultrasound-guided fine-needle aspiration (EBUS-FNA) was performed on an enlarged interlobar lymph node to rule out malignancy. A direct smear of the aspirated material was fixed in 95% ethanol and stained with a Papanicolaou stain. Variably sized yeasts were observed in aggregates focally forming unique geometric patterns in which mucinous strands appear between each organism (Figure 1). The diagnosis of Cryptococcus spp. was confirmed with a mucicarmine-stained cell block section in which mucinous strands can be seen highlighted (Figure 1D).
