Paragangliomas (PG) are rare neuroendocrine neoplasms with an annual incidence of 2–8 per million adults. PG are derived from paraganglia of sympathetic and parasympathetic nervous systems. Due to its rarity, PG pose a huge diagnostic dilemma equally for clinicians, radiologists and pathologists at unusual anatomic locations. Moreover, definitive diagnosis in limited tissue sample in fine needle aspiration cytology (FNAC) as well as biopsies is even more challenging. Herein, we report a case of peripancreatic PG diagnosed on endoscopic ultrasound guided fine needle aspiration cytology (EUS FNAC). 2 | CASE REPORT
