dyslipidaemia often co-exists with insulin resistance and we did not include it in our multivariable model in order to avoid overadjustment.1 Similarly, as we had already included history of myocardial infarction in our model, addition of coronary revascularisation could have created the same problem. Nevertheless, in order to directly address the concerns of Skelin and colleagues, we have added both variables to our model. As can be seen from the Table 1, this further adjustment made little difference to our findings.
