Nodular regenerative hyperplasia (NRH) is a rare condition characterized by diffuse, benign transformation of hepatic parenchyma into regenerative nodules, and is an under-recognized cause of portal hypertension. We present a case of NRH associated with the anti-PD1 agent pembrolizumab, a previously unrecognized complication of immune checkpoint blockade.A 35 year-old male with no history of liver disease was diagnosed with localized B-Raf mutant axillary melanoma, initially treated with anti-PD1 agent pembrolizumab.
