Genomic medicine is an emerging discipline that uses the patient's genomic information to deliver personalized medical care and promote health. Genomic analysis facilitates a molecular diagnosis in a subset of patients, which results in a precise understanding of why a patient develops a disease. This information is crucial to delivering the appropriate treatment, preventive medicine interventions, and family counseling to each patient. Here, we discuss the past, the present, and the future of genomic analysis in understanding and treating liver disease. We highlight the growing importance of implementing genomic medicine in the clinical practice of hepatology, the training of future hepatologists, and the advancement of liver pathobiology and novel non-invasive therapies. In 2022, we stand at an inflection point in the practice of hepatology. The utilization of genomic medicine is moving from niche to widespread, paving the way for expansion of genomics-informed hepatology clinical practice and research, a new era in molecular hepatology.
