We report one of the earliest prenatal observations of an accessory right inferior hepatic vein in a fetus with growth restriction at 36 weeks. Two‐ and three‐dimensional Doppler ultrasonography revealed this vein draining into the inferior vena cava below the right portal vein, without signs of porto‐systemic shunting or associated anomalies. This case contributes to the limited literature on hepatic venous system variants in antenatal life and underscores the role of detailed prenatal imaging in identifying uncommon vascular anatomies.
