On February 4, 2020, the Organ Procurement and Transplantation Network (OPTN)/United Network for Organ Sharing (UNOS) implemented the acuity circles (AC) policy.(1) Broader sharing of livers using concentric circles was developed to decrease variability in the median Model for EndStage Liver Disease (MMAT) at transplant. An AC policy carveout prioritizing marginal (eg, donation after circulatory death [DCD]) donors to patients on a waiting list at a transplant center within 150 nautical miles (NM) of the donor hospital was developed to address concerns that broader distribution may further the underuse attributed to longer travel distance and cold ischemia times.(13)
