Proposed algorithm for pretransplantation assessment of cardiovascular and pulmonary diseases. *Coronary CT angiography (with or without flow reserve assessment) may have a lower risk of nephrotoxicity, and thus, it can be a reasonable test to start with in LT candidates with high pretest probability for coronary artery disease or those with abnormal stress testing. However, if the patient had abnormal PET- CFR, it is reasonable to proceed directly to invasive coronary angiography given the superior performance of PET- CFR even when compared with coronary CT angiography as discussed in the article. **Examples are dobutamine stress echocardiography or positron emission tomography with coronary flow reserve. For selected patients with young age and without any risk factors for coronary artery disease, stress testing may not be necessary.
