An equine veterinarian reached out for advice regarding a horse he encountered with abnormal movements. The horse initially developed irregular, clonic contractions around the muzzle. For 2 years, the movements spread to the eye and ear areas. The horse was otherwise healthy and had a normal general physical examination. Laboratory investigations were unrevealing, and imaging was cost-prohibitive and therefore deferred. Video S1 captures repetitive, irregular asynchronous clonic spasms affecting the right side of the face and muzzle.
