Idiopathic normal pressure hydrocephalus (iNPH) is characterized by gait disturbance, cognitive impairment, parkinsonian signs, and urinary incontinence, responsive to shunt surgery. There is evidence that dopamine transporter imaging (DAT-SPECT) is reduced in 30%–60% of iNPH patients. The mechanisms underlying this finding are not fully understood, and it is not clear whether dopaminergic damage and parkinsonian signs in iNPH might be caused by the enlarged ventricular system, or if these INPH patients might have comorbidity with parkinsonism.
