We are grateful for Dr. Poliseli and colleagues' letter. Regarding their comments on the results of endoscopic third ventriculostomy (ETV) compared with ventriculoperitoneal shunt, we refer to the publications referenced here. The International Infant Hydrocephalus Study (IIHS) RCT very clearly addressed infants with aqueductal stenosis and looked not only into the surgical results (showing superiority of shunts below 6 months of age) but also at the neuro‐ developmental outcome at 5 years of age for both arms, showing similar results. As such, the IIHS serves as a model to evaluate the ETV versus shunt dilemma in other etiologies of hydrocephalus.
