After every major process safety incident, one question that invariably surfaces is “Why do we keep having incidents?” In this article and presentation, nine “I” factors will be presented as potential answers to this question. These factors, (which include inexperience, intentionality, and impermanence, among others) will also be analyzed for potential answers to the related question, “What can we do to break the incident cycle?”
