Some authors suggested preoperative color Doppler ultrasound to assess the hepatic vein and gallbladder bed to prevent vascular injuries to the dissecting plane, during a laparoscopic cholecystectomy [4]. In our view, to minimize the risk of iatrogenic injury to the middle hepatic vein terminal branch, special care should be taken during the last steps of dissection from the cystic plate, particularly when previous episodes of inflammation make the separation plane with the liver less obvious to identify.
