1. Pleuroperitoneal communication (sweet hydrothorax). 2. The most informative diagnostic method is peritoneal scintigraphy with technetium-99 m-DTPA. Computed tomography and magnetic resonance peritoneography are other diagnostic utilities. Previously used methylene blue test is not preferred due to decreased sensitivity and the risk of chemical peritonitis. 3. Pleuroperitoneal communication originates from the congenital or acquired defects of the diaphragm; elevated intra-abdominal pressure and hypertonic dialysate increase the risk of communication. The treatment options are conservative approach (interrupting peritoneal dialysis), chemical pleurodesis, surgical repair by thoracotomy, and video-assisted thoracoscopic surgery.
