Dear editor, I present you a clinical image of palmar fasciitis and polyarthritis (PFPA) in an 81-year-old woman. PFPA is an uncommon paraneoplastic syndrome that usually precedes the diagnosis of malignancy. Most patients are diagnosed with advanced cancer and have irreversible musculoskeletal symptoms. I think the image of the patient’s hands is an important clinical image because clinicians have to be aware of this unusual clinical sign. They have to recognise this clinical sign as an uncommon paraneoplastic condition and start cancer screening soon after presentation. I confirm that the clinical case has not been published before or is currently being considered for publication elsewhere. Thank you, in advance, for taking time to review this clinical case. Please contact me if you need any further information. Yours faithfully, Hannelore Celen
