Sexual transmission risk occurs in the context of serodiscordant condomless anal intercourse (CAI) where the seropositive partner is virologically detectable (VL+) and/or seronegative partner is not on PrEP. We analyzed correlates of serodiscordant CAI among 688 VL+ young men who have sex with men (YMSM). In multivariable analyses, serodiscordant CAI was associated with a receiving a HIV diagnosis in the past 6 months, greater depressive symptoms, and cocaine use during the past 90 days. Although HIV+ YMSM currently experience disparities across the continuum of care, those new to care may need support adopting risk reduction strategies with their sexual partners.ResumenEl riesgo de transmisión sexual a través del sexo anal sin condón (SAC) en parejas serodiscordantes occure cuando el compañero seropositivo es virológicamente detectable (VL+) y/o el compañero seronegativo no está en PrEP. Analizamos los correlatos de SAC con compañeros serodiscordantes en una muestra de 688 hombres jóvenes que tienen sexo con hombres (YMSM) viviendo con VIH y con una carga viral detectable. En análisis multivariables, el SAC serodiscordante estuvo asociado con un diagnóstico de VIH reciente (en los últimos 6 meses), síntomas depresivos, y consumo de cocaína durante los últimos 90 días. Aunque YMSM viviendo con VIH sufren disparidades en el continuo de cuidado, aquellos con un diagnóstico reciente pueden necesitar apoyo en la adopción de estrategias de reducción de riesgos con sus compañeros sexuales.
