Arising from neuroendocrine cells, neuroendocrine tumor (NET) is an increasingly diagnosed subclass of neoplasm that might occur throughout the gastrointestinal tract and pancreas. Liver metastasis is common especially in NET originating from the pancreas and small bowel and imposes significant detrimental influence on patients’ outcomes [1]. While resection of liver metastasis can result in favorable outcomes, majority of patients are not candidates for this modality of treatment because of bi-lobar disseminated involvement of the liver [2]. In such patients, orthotopic liver transplantation (OLT) has offered a promising therapy with favorable outcomes. While it is a controversial issue, most reports have emphasized that liver transplantation should be performed after resection of the primary tumor and without concomitant major surgical resection during transplant [3]. Here in, we describe 4 patients with pancreatic, small bowel, and mesenteric NET with diffuse liver metastasis that were treated with simultaneous OLT combined with the resection of primary tumor sites (Fig. 1).
