Anorectal malformations (ARMs) are common congenital abnormalities of the terminal hindgut, with an incidence of 1/2–5000 live births. In this issue of the Journal, Reddy et al. have described 54 neonates with ARM, of which, nearly half had a delayed (> 48 h) presentation, resulting in sig-nificant morbidity and high mortality. Children with delayed presentation not only had poorer perioperative outcomes but also inferior postoperative results. Some of them ended up having more extensive surgeries than what they would have warranted, if they had presented on time [1].
