Mesenteric cyst is rare; usually single, may be multiple. Mesocolic cyst is rarer. Few giant mesocolic cysts larger than 10 cm have been reported. Very few cases of mesenteric cysts presenting as adnexal mass are there in literature. Mesenteric cysts involving both ascending and descending and sigmoid mesocolons are not yet found among available database. Here is a unique case presenting to casualty with bleeding into ascending mesocolic cyst masquerading as right adnexal mass and being associated with infected giant mesocolic cyst of descending and sigmoid colons. Diagnostic incomprehension by clinical and ultrasonographic evaluation was clarified by an emergency laparotomy.
