Gastric volvulus is the rotation of all or part of the stomach more than 180° causing close loop obstruction. Total gastric volvulus is very rare and a life-threatening condition. Gastric volvulus is a clinical problem, when not recognized immediately, which can lead to necrosis. We report a 27-year-old young male who presented in the emergency department with pain in the abdomen, vomiting since 12 h, and having signs of peritonitis and was later diagnosed as acute gangrenous total gastric volvulus. Exploratory laparotomy and total gastrectomy with oesophago jejunal Roux-en-Y anastomosis was done. Postoperative period was uneventful and patient was started on oral feeds on the 10th post op day and was discharged.
