Significant incidental findings occur with cardiac imaging. Recently there has been growing interest in the utility of Tc-99m MIBI in the evaluation of small renal masses. In this case study, we describe the identification of a solid renal mass and its characterization with Tc99m MIBI. A 65-year-old female underwent myocardial perfusion imaging with Tc-99m MIBI in the setting of an elevated troponin. Upon review of the CAC/AC low dose CT, an incidental right renal mass was identified. Extended field post stress SPECT/CT demonstrated an absence of Tc-99m MIBI within the solid renal mass. Subsequent imaging and biopsy were performed confirming clear cell renal carcinoma. The patient successfully underwent a radical right nephrectomy. This case highlights that clinically significant incidental findings occur with cardiac imaging, as well as the utility of Tc-99m MIBI in the evaluation of small renal masses (Figures 1, 2).
