Light chains (AL) amyloidosis is a rare but lifethreatening disease. Cardiac involvement has the worst prognosis of any involved organ. Early identification of heart involvement may allow for prompt intervention and prevent irreversible myocardial remodeling. Fibroblast activation protein inhibitor (FAPI) imaging has recently been used in the evaluation of myocardial remodeling in several cardiac diseases. However, the clinical significance of FAPI imaging in AL cardiac amyloidosis remains unknown. Here we present a case with AL cardiac amyloidosis who had FAPI imaging, and compered with cardiac magnetic resonance (CMR).
