Platypnea-orthodeoxia syndrome (POS) is a rare syndrome in which hypoxemia and shortness of breath appear in the sitting position and improve to almost normal in the supine position. POS is the right-to-left shunt disease caused by coexistence of atrial shunt diseases and various structural disorders. Importantly, anatomic changes from supine to sitting position can cause preferential blood flow from the inferior vena cava (IVC) through the interatrial defect despite normal pulmonary arterial pressure [1, 2]. Here, we report precise findings from transesophageal echocardiography (TEE), right-to-left shunt flow-through PFO by postural provocation.
