Surgery offers the highest potential cure for a postoperative pelvic recurrence of colorectal cancer. However, a pelvic exenteration is often performed, which is highly invasive and its postoperative management is challenging. Although radiation therapy (RT) is a less invasive treatment option, the clinical outcomes of RT for pelvic recurrence of rectal cancer remain unsatisfactory with local control rates of 68% to 74%, even with stereotactic body RT (SBRT) for relatively small recurrent tumors.
