s / Atherosclerosis 263 (2017) e111ee282 e118 Aim: After myocardial infarction (MI), patients face a substantial risk of re-infarction. We hypothesize that MI promotes a systemic inflammatory response by inducing increased leukocyte supply from bone marrow and spleen. In this study, we set out to investigate the systemic inflammatory response in both the acute phase and 3 months after MI measured with 18F-DPA-714 PET/CT, a new tracer for TSPO receptor. Methods:We performed an 18F-DPA-714 PET/CT and assessed the number and phenotype of leukocytes and hematopoietic stem and progenitor cells (HSPCs) by flow cytometry in 7 patients in the acute phase of a MI (up to 18 days after the event) and after 3 months and compared themwith healthy controls. Results: Patients in the acute phase were characterized by a leukocytosis and increased number of circulating HSPCs (13,940[10,505-18,232] HSPCs/ml versus controls: 41[30-47] HSPCs/ml, p1⁄4<0.001), which persists after 3 months (10,360[6,935-14,070] HSPC/ml, p1⁄40.31). Furthermore, we demonstrated increased 18F-DPA-714 uptake in the acute phase in bone marrow (SUVmax 7.78±2.0 versus controls SUVmax 5.08±1.5, p1⁄40.016) and spleen (SUVmax 5.5±1.3 versus controls: SUVmax 4.1±0.7, p1⁄40.039). After 3 months 18F-DPA-714 uptake in the spleen persists, (SUVmax 5.7±1.5, p1⁄40.67) in contrast to the bone marrow, where 18F-DPA-714 uptake is normalized again (SUVmax 5.73±1.1, p1⁄40.011). Conclusions: Patients have a prolonged inflammatory state after MI, characterized by increased numbers of circulating leukocytes and their precursors. 18F-DPA-714 PET/CT-scan showed MI triggered hematopoietic organ activation as instigator of the prolonged leukocyte oversupply. This inflammatory response could be an interesting target for new treatment options to prevent recurrence of events. PO027.
