A 72-year-old man with supra-renal aneurysm and two large penetrating ulcers (PAU) in the descending aorta was referred for endovascular treatment. To avoid long segment aortic coverage and the attendant risk of spinal ischemia, combination of two different stent technologies was utilized. A multi-layer flow modulator was implanted in the thoracic aorta for the PAU, followed by implantation of a customized four-vessel fenestrated stent graft for the supra-renal aneurysm. The patient remained well at two years with CT evidence of exclusion of the supra-renal aneurysm and involution of the PAU.
