The mainstay of symptomatic treatment is to perform a therapeutic thoracentesis. The procedure, like any other procedures, carries its own risks and benefits. Ideally, complete drainage of the pleural effusion is desired, however, an uncommon complication of therapeutic thoracentesis is re-expansion pulmonary edema (RPE). This is a potential life-threatening complication that can occur at any time in the first 24 hours and can also include non-specific symptoms of dyspnea, tachypnea, cough, chest discomfort. We describe an unfortunate case of RPE after a therapeutic thoracentesis.
