INTRODUCTION:Multifocal cavitary lung lesions represent a diverse collection of pathology including infectious, autoimmune, vascular, and malignant disease. Radiographic features such as timing of appearance, distribution, wall thickness, and association with nodules or ground glass may lead one to favor one diagnosis over another. Still, all lung lesions should raise the suspicion for cancer, especially in older patients with an indolent presentation. While we tend to think of primary lung cancer as a solitary mass and associate a patchy disseminated pattern with metastatic disease, invasive mucinous adenocarcinoma (IMA) is a primary lung cancer that frequently presents multifocally, and is known to cavitate.
