METHODS: Between January 1, 2017-December 31, 2018, there were 318,508 tweets using the hashtag #burnout, but relatively few were related to healthcare. We identified two hashtags commonly used by healthcare providers on Twitter and analyzed data using Symplur Signals, LLC: #StopICUBurnout, created by CCSC and an organically driven #PhysicianBurnout. RESULTS: There were 14,971 tweets by 9,111 users for #PhysicianBurnout as compared to 1,744 tweets by 811 users for #StopICUburnout. There was a significantly higher number of monthly tweets (n1⁄4624 874 vs. n1⁄473 160; p1⁄40.006), monthly users (380 541 vs. 34 52; p1⁄40.005), and monthly impressions (n1⁄42,984,172 4,253,666 vs. n1⁄4166,270 443,026; p1⁄40.004) for #PhysicianBurnout vs. #StopICUburnout. About one third of the users of both these hashtags were physicians (33% vs. 32%). Although #StopICUburnout is an organizational hashtag, more users of #PhysicianBurnout were organizations (19% vs. 10%). #StopICUBurnout usage increased during CCSC conferences, while #PhysicianBurnout had more sustained usage throughout the year. Network analysis showed less connection between users of #PhysicianBurnout. Among the top hashtags used in associated with #PhysicianBurnout, 3/10 were associated with electronic health records (EHR); (#EHR, #EMR, #healthIT) while 4/10 for #StopICUBurnout were conference or society hashtags (#CCC47, #ATS2018, #SCCM, #pulmcc). CONCLUSIONS: We found that #PhysicianBurnout had more total users, monthly individual and organizational users, more tweets and more sustained usage as compared to the hashtag promoted by CCSC, #StopICUburnout. Discussions about EHR were commonly associated with #PhysicianBurnout, highlighting the importance of this stressor. Organizational hashtags have shorter high engagement periods during annual conventions while organically driven hashtags created by the twitter community are more sustainable. CLINICAL IMPLICATIONS: Recently, awareness of burnout in critical care has tremendously increased. The Twitter footprint of conversations circling burnout in healthcare could be reflective of this increasing awareness at both the individual and organizational levels in the critical care community.
