TOPIC: Allergy and Airway TYPE: Medical Student/Resident Case Reports INTRODUCTION: Idiopathic thrombocytopenic purpura (ITP) is an acquired bleeding disorder, characterized by thrombocytopenia due to autoantibodies against platelet antigens. The pathogenesis is not clearly understood;however, it is thought to be secondary to an inciting event. These events might include systemic conditions or infections. These infections are typically of viral nature, including human immunodeficiency virus, hepatitis C, cytomegalovirus, varicella zoster virus or coronavirus. In addition, ITP has been rarely described in literature after undergoing viral vaccination. CASE PRESENTATION: This is a case of a 61-year-old male patient with history of hypertension and SARS-CoV2 viral infection 5 months earlier, who came to our institution after developing small violaceous spots throughout his body, with associated fever, chills, general malaise, and dark urine, 3 days after receiving the second dose of the mRNA-1273 COVID-19 vaccine. He denied nausea, vomits, rhinorrhea, shortness of breath, cough, chest pain, abdominal pain, hematemesis, hematochezia, melena, or recent illness. Vital signs were temperature of 98.4 F, pulse in 68 bpm, respiratory rate in 18/min and blood pressure in 156/74 mmHg. Laboratories showed leukocytes in 4.9 x 10
